A Home Care Package (HCP) provides tailored, coordinated, and ongoing support to help you continue living independently in your own home. It’s designed around your needs, giving you the flexibility to receive the care and services that matter most to you.

About Us

Celebrate Aged and Community Care has proudly delivered Home Care Packages to the Glamorgan Spring Bay Municipality since 1987, Dorset Municipality since 2012 and Break O’Day more recently in 2024.

All our packages are delivered with a Consumer Directed Care (CDC) approach, which puts you – the consumer – in the director’s chair. We work with you to identify meaningful goals that enhance your health, wellbeing, and connection to family, friends, and the wider community.

What is a Home Care Package?

Home Care Packages are designed for older Australians with more complex care needs than what the Commonwealth Home Support Programme can provide. They offer a coordinated approach to support with daily tasks, personal care, health, and wellbeing – all delivered in the comfort of your own home.

The Australian Government subsidises these services, and what you pay depends on the services you receive and, in some cases, your financial situation.

Services You Can Receive

Depending on your package level, support may include:

  • Services to keep you well and independent – personal care, nursing, allied health

  • Services to keep you safe at home – cleaning, home maintenance, modifications, assistive technology

  • Services to keep you connected to your community – transport, social supportOur ILUs are ideally located close to the beach, shops, bowling and golf clubs, and within easy walking distance to cafés and restaurants. It’s the perfect balance of convenience, community, and coastal living.

How to Get a Home Care Package

  1. Make an appointment with our Home Care team to discuss your needs.

  2. Assessment via My Aged Care – an ACAS (Aged Care Assessment Service)  assessment may be required to determine your eligibility.

  3. Receive your Home Care Package and work with our team to create a personalised care plan.

  4. Ongoing review and support to ensure your care continues to meet your goals and needs.
